Progress V9, adding a new menu / a new button to the toolbar

Hi
I am programming in V9.1c. I am currently trying to add a menu or a button to the default toolbar dyntoolbar. I have used knowledge base 21183 and 19551 , but still no joy.
I have added the code into actioncustom.p and toolbarcustom.p and I have added the,m as super procedures.
Any ideas would be greatly appreciated
